বিভাগ, জেলা প্রভৃতি ধাপ পার হয়ে ওয়ার্ড পর্যন্ত নির্বাচন করার পর স্ক্রিনে একটি অনলাইন ফরম আসবে। অনলাইন জন্ম নিবন্ধনের আবেদন ফরম প্রথমে বাংলায় (ইউনিকোড) ও পরবর্তীতে ইংরেজিতে পূরণের পর প্রয়োজনীয় সম্পাদনা করে সংরক্ষণ বাটনে ক্লিক করতে হবে। সংরক্ষণ বাটনে ক্লিক করলেই আবেদন পত্রটি সংশ্লিষ্ট নিবন্ধক কার্যালয়ে স্থানান্তরিত হয়ে যাবে, আবেদনকারীর আর কোনো সংশোধনের সুযোগ থাকবে না। এরপর পরবর্তী ধাপে প্রিন্ট বাটনে ক্লিক করলে আবেদন পত্রের মুদ্রিত কপি পাওয়া যাবে। সনদের জন্য ১৫ দিনের মধ্যে উক্ত আবেদনপত্রের মুদ্রিত কপি ও প্রযোজ্য ক্ষেত্রে প্রয়োজনীয় প্রমাণপত্রের সত্যায়িত কপিসহ নিবন্ধক অফিসে যোগাযোগ করতে হবে।
