• First, go to your internet browser and enter the https://bdris.gov.bd/br/application website.
• There, you’ll find three options which are birth address, present address, and permanent address. Select whichever you want to apply from and click next.

• Once you fill up all the empty boxes, you should re-check all the information.  After that, you can click on next.
• That will submit the application and open the entire application form in front of you. You need to print it off using a printer.

ওয়েবসাইটে প্রবেশ করার পর নিচের স্টেপ গুলো মনোযোগ সহকারে ফলো করুন।

স্টেপ -০১

ওয়েবসাইটটিতে প্রবেশ করার পর উপরের ছবির মতো একটি ওয়েবসাইট ওপেন হবে। ওয়েবসাইটটি জন্ম নিবন্ধন বিষয়ক হওয়ায় এখানে জন্ম নিবন্ধনের উপর বিভিন্ন সেবা দেখতে পাবেন।

আপনি ওয়েবসাইট হেডারের ‘জন্ম নিবন্ধন’ মেনু থেকে ”জন্ম নিবন্ধন সনদ পুনঃমুদ্রন” মেনুতে ক্লিক করুন।

স্টেপ -০২

দ্বিতীয় ধাপে এমন একটি পেজ ওপেন হবে, যেখানে লেখা থাকবে ’জন্ম নিবন্ধন এর প্রতিলিপির জন্য আবেদন।’ এর নিচে দুটি খালিঘর রয়েছে।

খালিঘর গুলোতে জন্ম নিবন্ধন নম্বর এবং জন্ম তারিখ দিতে হবে। এটি অবশ্যই আপনার হাতে লেখা জন্ম নিবন্ধন থেকে হুবহু কপি করে লিখবেন। জন্ম নিবন্ধন নম্বর অবশ্যই ১৭ ডিজিটের হতে হবে। উদাহরণস্বরূপঃ জন্ম নিবন্ধন যাচাই 19860915428117351 নম্বর এমন। নম্বর লিখতে গিয়ে কোনো প্রকার ভুল করা যাবে না।

সঠিক তথ্য দিয়ে “অনুসন্ধান” বাটনে ক্লিক করলেই আপনার আইডি নম্বর, জন্ম তারিখ, নিজের নাম ও বাবা -মায়ের নাম দেখতে পাবেন।

উল্লেখ্য, অনেকের ক্ষেত্রে এমন হয় যে, জন্ম নিবন্ধন নম্বর এবং জন্ম তারিখ দেওয়ার পরও কোনো রেজাল্ট প্রদর্শন করে না। এটি মূলত ভূল তথ্য প্রদানের কারণে এমন হয়।

আপনার জন্ম নিবন্ধনটি হাতে লেখার কারণে অনেক সময় লেখায় ভুল তথ্য উঠে যায়। যে কারণে ওয়েবসাইটের ডেটাবেজে থাকা তথ্যের সাথে হাতে লেখা জন্ম নিবন্ধনের তথ্য মিলে না।

এতে বিচলিত হওয়ার কোনো কারণ নেই। ভুল তথ্যের কারণে আপনি হয়তো নিজে অনলাইনের মাধ্যমে জন্ম নিবন্ধন ডিজিটাল করতে পারবেন না। তবে আপনি যদি হাতে লেখা জন্ম নিবন্ধনটি সংশ্লিষ্ট ইউনিয়ন পরিষদ অথবা সিটি কর্পোরেশনের কার্যালয়ে নিয়ে যান তবে তারাই আপনাকে সঠিক তথ্যের মাধ্যমে আপনার জন্ম নিবন্ধনকে ডিজিটাল করে দেবে।

আমারটা যেহেতু তথ্যের ভিত্তিতে রেজাল্ট প্রদর্শন করেছে, সেহেতু আমি “নির্বাচন করুন” বাটনে ক্লিক করে পরবর্তী স্টেপে চলে যাব। অবশ্যই স্টেপগুলো ফলো করুন।

স্টেপ -০৩

তৃতীয় ধাপে আপনাকে কিছু তথ্য দিতে হবে। যেমন আপনার দেশ, বিভাগ, জেলা, উপজেলা / সিটি কর্পোরেশন, ক্যান্টনমেন্ট, পৌরসভা / ইউনিয়ন, অফিস, নাম, ঠিকানা, মোবাইল নম্বর এবং ইমেইল ইত্যাদি।

এখানে একটি লক্ষ্যনীয়, আবেদনকারী যদি আপনি নিজে হন তবে “নিজ” সিলেক্ট করবেন। আর যদি আপনার জন্ম নিবন্ধনের আবেদরকারী অন্য কেউ হয়, তবে যথাপোযুক্ত ঘর সিলেক্ট করে তার জন্ম নিবন্ধন নম্বর এবং জাতীয় পরিচয়পত্র নম্বর দিতে হবে।
তারপর আবেদনকারীর মোবাইল নম্বর দিতে হবে। আবেদনকারীর যদি ইমেইল / জিমেইল থাকে তবে খালিঘরে দিয়ে দেবেন, আর যদি ইমেইল / জিমেইল না থাকে তবে Email -এর ঘর ফাকা রাখবেন।

সঠিকভাবে তথ্য দেওয়ার পর “সাবমিট” বাটনে ক্লিক করুন।

স্টেপ -০৪

আপনার ডকুমেন্টটি সাবমিট হয়ে গেলে চতুর্থ ধাপে উপরের ছবির মতো একটি পেজ ওপেন হবে। এই পেজে কিছু তথ্য দেখাবে। যেমন, শুরুতেই দেখাচ্ছে ’আবেদনটি সংশ্লিষ্ট কার্যালয়ে দাখিল হয়েছে।’ তারপর আপনার আবেদন পত্রের নম্বর সহ আরো কিছু কথা বলা রয়েছে।

আবেদন পত্রের নম্বরটি খুবই গুরুত্বপূর্ণ। এটি পরবর্তী সময়ে কাজে লাগবে। তাই যত্ন করে খাতায় লিখে ফেলুন।

নিচে “আবেদন পত্র প্রিন্ট’ নামের একটি বাটন দেখতে পাচ্ছেন। এখানে ক্লিক করলে আপনার জন্ম নিবন্ধন অনলাইন কপি ডাউনলোড হয়ে যাবে pdf আকারে। এটিকে প্রিন্ট করে নেবেন।

আবেদনপত্রটি প্রিন্ট করে আপনার সংশ্লিষ্ট নিবন্ধকের কার্যালয় অর্থাৎ ইউনিয়ন পরিষদ অথবা সিটি কর্পোরেশন অফিসে জমা দিতে হবে। কোনো কারণে যদি আবেদনপত্রটি প্রিন্ট করতে না পারেন তবে আবেদন পত্রের নম্বরটি উক্ত অফিসে জমা দিতে হবে।

ইউনিয়ন পরিষদ অথবা সিটি কর্পোরেশন অফিসে আপনার আাবেদন পত্রটি জমা দেওয়ার পর তারা আপনার আবেদনপত্রে প্রয়োজনীয় স্বাক্ষর এবং সীল দিয়ে দেবে।

এজন্য আপনাকে নির্দিষ্ট পরিমাণ ফি দিতে হবে। আবেদনপত্র জমা ও ফি প্রদান করার মাধ্যমে কর্তৃপক্ষ আপনার এনালগ জন্ম নিবন্ধনকে ডিজিটাল জন্ম নিবন্ধনে কনভার্ট করে দেবে।
